Procedure for using rights to receive reward for those who buy the program through online channels or over the counter at the hospital.

1. The hospital will send an SMS confirming the right to receive the reward at the end of the month you had your health check-up. 
2. Coupons may be collected at Digital Corner. Please inform staff to receive coupon: 
   • 1st Floor, Building 1, Classic wing, Samitivej Sukhumvit 
   • 1st Floor, in front of the Internal Medicine Department, Samitivej Srinakarin 
3. Show your coupon at the service point. 
4. The hospital is not responsible for lost or damaged coupons 
5. Coupons may not be returned, exchanged or converted to cash 
6. Coupon expires June 30, 2022
