Receiving the Moderna Vaccine at Samitivej Sukhumvit and Samitivej Srinakarin Hospitals

The queue order for receiving the vaccines has been organized in accordance with the urgency of the patient’s needs, as follows:

Group 1: Those who have never received a COVID-19 vaccine 

Group 2: Those who require a second dose or booster vaccine are considered along with how long ago they received their vaccine and in line with recommendations regarding the Moderna vaccine (image below). 


 

Moderna vaccine scheduling

Samitivej has upgraded its vaccine management system, so you can now check your vaccine appointment, make an additional appointment, or make changes in advance at https://smtvj.com/3md8yn8. The hospital will send a confirmation SMS at least 3 days before your appointment. We will vaccinate people according to their selected appointment date. If your scheduled appointment date has passed, you can make a new appointment by selecting a free queue time in the system here: https://smtvj.com/3md8yn8.

The hospital will update customers periodically regarding their vaccine appointment, as the Moderna vaccine is imported in batches. You can check your appointment details in our system at any time and make additional appointments or postpone your appointment here: https://smtvj.com/3md8yn8.

Transfer of privileges

Because the hospital is serving as representative of vaccine recipients in reserving vaccines through the Government Pharmaceutical Organization, these vaccines belong to the specified vaccine recipients only and the hospital may not cancel or provide refunds for the vaccines. 

  • Vaccine recipients may log into the system and transfer vaccines rights themselves, but this should be done before confirmation of vaccination history and personal health information has been provided. Upon successful transfer of vaccines rights, transferee will receive an SMS notification inviting them to log into the system to confirm their own vaccination history and health information.   • In the event that it is not possible to make a transfer via the system, click here to request help from our staff through this online form. 
  • The Moderna vaccine is about to reach its expiry date (final vaccines will be administered on September 14, 2022 at Samitivej Sukhumvit Hospital and on September 30, 2022 at Samitivej Srinakarin Hospital). Please make an appointment or state your intention to donate your vaccine by August 20, 2022 (Appointment scheduling will be closed after August 21). If you do not make an appointment or inform us of your donation on the specified date, we reserve the right to cancel or refund your purchase. Appointment postponement

Once a vaccine recipient has received their Moderna vaccination appointment via SMS, if they are not available at that date and time they should click here to request a postponement through our online form no more than 3 days after they receive the SMS. This is because the Moderna vaccine is about to reach its expiry date (final vaccines will be administered on September 14, 2022 at Samitivej Sukhumvit Hospital and on September 30, 2022 at Samitivej Srinakarin Hospital).     • Coma* caused by COVID-19 vaccinations: sum insured 1,000,000 baht
    • Medical expenses for inpatients due only to complications or side effects* of COVID-19 vaccinations: sum insured 100,000 baht per year
    • Total permanent disability, dismemberment, or loss of sight due only to complications or side effects of COVID-19 vaccinations: sum insured 500,000 baht

*Coma, brain death, neurologic failure or terminal illness which a doctor diagnoses as incurable and/or which lead to death from coronavirus COVID-19 vaccinations.
**Severe allergic reactions to COVID-19 vaccines include chest tightness, breathing difficulties, severe headaches, vomiting (more than 5 times), stroke-like symptoms (e.g. lip drooping and leg or arm weakness), bruises or bleeding spots, skin rash, seizure and unconsciousness.

    3. Receive your vaccination at Service Points 3-7 at Bancha Lamsam Auditorium, Building 2, 6th floor 

Note: Service Points 1-2 will be moving to Bancha Lamsam Auditorium, Building 2, 6th floor on January 6, 2022. 

  • Samitivej Srinakarin  
    1. Bring your passport to your appointment 
    2. Register and receive your vaccination on the 4th floor
