Documents Required for Issuing Confirmation Letter:

•    Passport copy of patient

•    Entourages’ passport (s) copy {not exceeding 3 persons}.

•    Updated patient’s medical information/ reports

•    Affidavit of Support for patient's companion with signature. Please revert back to us once completed.
After receiving the above documents, Samitivej Hospitals will issue a Confirmation Letter and submit it to The Ministry of Public Health to request permission for the patient and entourages to enter Thailand. Once approved, we will send the approved Confirmation letter to the patient to submit to the Royal Thai Embassy in your local residence for a Certificate of Entry (COE).

Documents Required to Enter Thailand:

•    Certificate of Entry (COE) issued by The Royal Thai Embassy in your local residency.

•    Fit to Fly certificate from a healthcare institution within 72 hours before departure.

•    COVID-19 free certificate and result within 72 hours before departure.

•    Financial evidence stating financial ability to bear medical expenses in Thailand.

•    Health insurance with minimum coverage of 100,000 USD for treatment of COVID-19 and other conditions while in Thailand.

•    Letter from the referring hospital in your country, stating reasons for medical treatment in Thailand.

•    Affidavit of Support for patient’s entourages with signature. Please revert back to us once completed.

•    Confirmation letter for AHQ (Prepared by Samitivej).
